Input Fields:
Title		A title for these calculator results that will help you identify it if you have printed out several versions of the calculator.
Cost per Pack		The amount you pay for each pack of cigarettes.
Sales Tax		The sales tax percentage you will pay on this purchase.
Cigarettes per Pack		The number of cigarettes in each pack.
Smoked per Day		The approximate or average number of cigarettes you smoke each day.
Show Cost over		The specific number of years, months, weeks, or days you would like to calculate smoking costs for.
Output Fields:
Taxed Cost per Pack		The actual cost for a pack of cigarettes after tax has been added.
Cost per Cigarette		The amount you are paying for each cigarette.
Time		Smoking costs have been calculated for each of the time periods shown in this column.
Cost		The total cost for cigarettes over the time period shown.
Cigarettes		The total number of cigarettes smoked over the time period shown.
